Our Story – A Community Based Solution
In May of 2011, leaders from organizations participating in the Central Oregon Health Council came together around the promise that we can – and must – serve patients with complex health issues better.
As a result, a community work group formed to research successful healthcare models nationwide and recommend a collaborative solution.
The group discovered a need to improve care coordination among patients with complex health issues. Not only would this coordinated care strategy improve patient care, it would also support providers in delivering care, reduce overall health care costs and improve population health. The group also confirmed that Central Oregon already had incredible resources, organizations and individuals committed to improving the community’s health.
Combining this research – and the existing resources within the community – the work group began the development of Bridges Health, an innovative program designed to treat complex health conditions and barriers to wellness through better care coordination.
